Has anyone seen this problem?!

Looking for help for my sister in Florida with her Beautiful 2 year old black lab named Ranger. He has not been neutered.
(Picture of the notes is a summary from my sisters diary of what has been done for him)
It just started in January of this year. He has been losing hair around his eyes in little areas around his nose though now itís mostly around his eyes. In that second picture you can see incredible swelling which is the pink areas that you see.
He has started scratching at his face and his underbelly.
They donít use any sprays or powders on him. He is on Trifexus for flea and heartworm protection. Although she has not given that to him last month or this month yet.
The last thing he was on was the prednisone which helped amazingly. But of course he cannot be on that too long and just recently came off of it and once again looks bad as in the pictures from yesterday.
Doctor says at this point Heís going to treat him as an auto immune disorder. But they will start with some blood work on Thursday even though he is on steroids. They need to start somewhere to get a baseline on him. Wondering if anybody might know what this might be and allow my sister to move in the right direction or ask the right questions to the vet? Anything any comments or ideas are welcome. Thank you thank you thank you God bless!

I see an Alaphia River AH- and Boyette- neither one of them have an ophthalmologist-
that would #1 be my first step.
Prednisone is great about relieving symptoms but it won't cure anything ever and will cause numerous side effects... it looks like she's in Lithia area- there are board cert ophthalmologists near enough to see. ACVO Public

I'm not sure I wouldn't see a dermatologist too- and actually if she could get in @ UF w the ophthalmology service, the derm service there is wonderful too so it might be a one stop shop if it happened ophth thought derm was a good idea and could call over given the drive...

Yes true, I guess with all the hair loss around the eyes and the nose they were just focusing on that. Great idea Iím surprised the vet did not recommend that also.

Demodectic mange starts around the eyes and nose....

Very worst thing is having a dog with mange on immune suppressants...

And that would make sense if the vet was thinking he might have an auto immune disease since this mange affects dogs with that. Thanks so much
