There's a discussion on my other board about what information you put on tags. Very interesting points being brought up.
I never put my dogs' names or my address on tags. I don't want anyone knowing their names or where I live. So on one side I have a medic alert symbol, (everyone's familiar with it and they don't even have to know how to read ), and the other side has only two phone numbers and "I'm microchipped" on it. No address and no names on the tags. They're not necessary, IMO.

"I'm Micro Chipped"
I don't understand the "no name" reasoning, if they are close enough to my dog to read his tag, they already have my dog. If someone wants to steal my dog, not knowing his name won't stop them. I don't consider his name personal information.
Name on one side and mine and my wifes cell numbers on the other side. I want their names on them so if they get picked up someone caring for them can help comfort them by knowing their names.

NEEDS MEDICATION (one of my two dogs actually does need medication, and the other has a gene mutation where he cannot have certain meds, so I figure either way this does help people know there are medical needs they need to be aware of)
I do REALLY like the "Reward if Found" idea - next time I order new collars I'll have to figure out a way to get that on there, maybe instead of the microchipped line. I order my collars from gun dog supply, and they attach ID plates for free (which I love because they don't dangle and jingle-jangle).
Bella has a separate yellow tag on her collar with her microchip ID, so I don't feel the need to have that on her name tag. It's the one that Home Again sends for free.
We have some redundant systems. On the collar itself there is my cell#, my home#, the town we live in. This is a brass plate attached to the collar itself from gundogsupply.com.
Then, on a carabiner around the collar, we have a microchip tag. Next I have a Cookie Monster tag with my address and cell#, and of course Cookie's name. If needed, one could find me by either the rabies number, dog license#, or the ID tag she wears to allow her to hike at one of the privately owned reservations near here.
Knock on wood....I've never lost Cookie, but I DID lose her tags on a hike once. A nice guy mailed them back to me.
I'm not allowed to bring phone into the building at work, but could list my work phone number as well. Which is a good idea, I may have to add that!
I guess I'm lucky that I have friends and neighbors that would pick up my dog if I was unavailable. Even my friends that don't like/don't have dogs would get them as a favor to me, even if it meant keeping them in a garage or even just putting them in my house.
