Update to Problematic spay

About six months ago I posted about the problem my sister was having with her young female rescue

This post is for my sister who has a year old rescue female (not a Golden). They got the dog when she was a young pup and she had already been spayed. Last Friday she presented with a swollen vulva and my sister said she has been very humppy recently. They took her in and the vet completely missed it and said what they were seeing was dirt or an ingrown hair? Then on Sunday my sister noticed bright red blood on her hair under her vulva, she cleaned her off but the blood kept returning. At this point she contacted me.

My Selli was intact until she was six years old so we went through five or six heat cycles and she developed pyometra at the end of her last heat and she had an emergency spay.

I told my sister that sometimes some uterine or ovary tissue gets missed in a spay and the dog then will go into heat and that if this is so they will need to be careful about the possibility of a stump pyometra. But in my opinion and from the symptoms it sounded like she was in heat. We discussed a possible UTI but she doesn't have the other symptoms of a UTI.

They took her in on Monday and she got an ultrasound and the vet thought he saw evidence of a stump pyometra so she went in for surgery but they did not find any uterine tissue (my sister said they found no reproductive tissue) and no infection.

So now they are treating her with antibiotics for an unidentified infection. Once again, her main symptoms are a red swollen vulva, periodic bright red blood below her vulva and increased humping.

The vets have not done blood work or taken an urine sample.

Is it possible that part of an ovary was left and it is producing the hormones to go into heat?
She had emergency surgery but they did not find any reproductive tissue. Now it is six months later and she is having the same symptoms. All I can think of is that there must be some tissue left, but no one wants to put her through surgery again. Aside from the annoyance of having to keep her clean and dealing with their neutered male sniffing her all the time, is there a danger if she retains some reproductive tissue?
